Why Do Vaccines Fail?

We do occasionally come across vaccine failure in practice. How do you explain this to a client who already has some doubt about the safety of vaccines after the recent press stories?
There are many possible reasons.
Vaccines sometimes made up incorrectly, storing incorrectly, using out-of-date stock.
Persistance of maternally derived antibodies, vaccinating a comprismised/immuno-suppressed animal and so on are the main reasons.
Any vaccine reaction/failure should be thoroughly reported including batch numbers used and backing the reports with any lab results.
